Experience journey maps help us model how different users / customers implement care delivery best practices.
How might we map the evidence-based and standardised care practices, so that we can define how and where to best intervene along a patient’s care journey, so that we can lead them to the best possible outcome?
At Philips we added a new artefact and methodology - Care Pathways - to our toolkit of healthcare domain strategic thinking.
Care Pathways are used to map a patient’s end-to-end journey across the health continuum, from prevention through to recovery or self management. They capture evidence-based care interventions that should take place at each stage to achieve the designed health outcomes.
These interventions include activities delivered by members of a wider interdisciplinary team such as physicians, nurses, pharmacists, physical therapists, social workers and informal caregivers.
Clear picture and story of how Philips’ portfolio support a given pathway and what outcomes they contribute to.
Identification of key pressure points and gaps in a pathway where health technology solutions could add value, this informing the innovation roadmap.
Improved understanding if local needs and ways to drive improvements for customers towards clear outcomes.
Established the methodology, tools and governance models to create and maintain Care Pathways across the organisation,
Delivered up to 5 Care Pathways across the Cardiology, Respiratory healthcare domains.
